Adult Day Services (ADS) is a licensed not-for-profit, multi-ethnic community-based program offering nursing/health care, restorative therapies, social services, and recreational activities for adults with potentially debilitating conditions.

Located at the Friends House facility in northeast Santa Rosa (see map), Adult Day Services helps frail older adults as well as younger, functionally impaired adults remain independent as long as possible through:

Improving and maintaining each participant's health and functional abilities to optimum level
Delaying or preventing premature placement in a skilled nursing facility or other institution
Enhanching the quality of life for each participant as well as their caregiver

Service include:
Nursing care
Rehabilitative therapies
Social services
Recreational and social activities
Nutrition and dietary counseling
Psychological consultation

Transportation is provided to and from ADS when needed. Our service area includes primarily Santa Rosa and Windsor. Transportation for participants from outlying areas such as Sebastopol, Forestville, Rohnert Park, Oakmont, and Healdsburg can also be arranged. Otherwise, caregivers provide transportation.

Costs are covered either through MediCal for those who are eligible or through private fees determined by the number of days per week the program is attended.

ADS also offers financial assistance to caregivers through grant funding provided by the Area Agency on Aging. These funds are intended to support caregivers by offering valuable respite for them, while providing their care receivers with on-site program services. In order to qualify for these funds, applicants must be the primary in-home caregiver for a care receiver who is 60 years old or greater. The care receiver must also have cognitive deficits - such as memory loss, problems doing familiar tasks, changes in personality, trouble knowing times, dates, or places - and/or deficits with their daily living activities - such as difficulty bathing, dressing, grooming and overall self care.

For more information about Southwest Adult Day Services, program costs, and MediCal eligibility, email ADS or call (707) 573-4522.
