SCHC offers Mental Health Services for our established health center patients. Mental health professionals are available on an on-call basis or by appointment with a referral by a primary care physician. Because mental health care is largely unavailable and unaffordable to people who are uninsured or on Medi-Cal, SCHC created this program to meet the mental health needs of our patients.
On-site, integrated mental health services enable SCHC to provide more comprehensive care for our patients should they need a medication evaluation by a psychiatrist, or psychotherapy by one of our therapists, in those cases where these services can’t be provided by the primary care physician.
Psychotherapeutic services are provided for individual patients of all ages, couples and families. At present, the Mental Health Department offers one mental health support group for English speaking patients on Fridays, lead by Colleen Krinard, LCSW. Several other groups are offered for Spanish speaking patients.
Meet the SCHC Mental Health Team
SCHC's seven-person mental health services team is under the leadership of Dr. David Anglada-Figueroa and features three registered psychologists, three licensed psychologists, and a mental health intern. All are bilingual and bicultural.

How Integrated Primary Care Works
On-call therapists are available at both the Lombardi Court Health Center and Chanate Family Practice Center:
Mondays: 2:30-5 pm
Tuesdays through Fridays:
10 am-12:30 pm and
2:30 pm-5 pm
If a physician identifies a patient potentially in need of mental health services, they can contact the on-call therapist to talk with the patient immediately by phone or in person. This is known as integrated primary care, designed to provide immediate mental health resources to health center patients. Integrated primary care also ensures close communication between medical providers and mental health professionals to enable effective, coordinated treatment.
The on-call therapist meets with the patient for a 15-30 minute session that includes evaluation, assessment, and possible intervention. This allows the therapist to determine whether the patient needs immediate intervention or longer term, specialty therapy - usually a series of up to 10 appointments.
The hallmark of integrated primary care is that the therapist and physician work together closely. The patient's primary doctor always leads the decision making about the patient's overall treatment.

If you are an established SCHC patient and you would like to know more about mental health services available to you, check with your family doctor or provider.
SCHC mental health services are covered by MediCal, CMSP, or on a sliding scale basis* to uninsured patients. Services are not available to those with private insurance.
If you are not a regular patient at SCHC and you need mental health services or more information, you must first make an appointment to see a medical provider by calling 707.547.2222.
* The cost of SCHC mental health services program is supplemented through funding from California's Mental Health Services Act (MHSA). Passed in 2004, this act provided increased funding and resources to support county mental health programs. SCHC is one of several community organizations partnering with the County of Sonoma to provide prevention and early intervention to underserved and/or uninsured adults, youth, and children.
More About Our Mental Health Providers
Rafael Flores-Jimeno provides counseling services at Elsie Allen Health Center every Friday. He is a Marriage and Family Therapy Intern under the supervision of Dr. Rodolfo Rodriguez. Rafael has worked in the mental health field since 1979. He specializes in adolescent care, bilingual individual and family therapy, substance abuse, and clinical work. Peruvian by birth, he has lived in the United States since 1976 with one five-year interlude in his home country when he worked for a private university and ran a private practice.
