The Southwest Community Health Center Outreach Team's job is to let the community know about SCHC services including our Chanate Health Center, Elsie Allen Health Center, and Southwest Adult Day Services at Friends House. These folks spends a lot of time out in the community at local schools, Elsie Allen Health Center, Santa Rosa Junior College, youth clubs like Chops, homeless shelters, and at a variety of public meetings to educate the public. SCHC's outreach and education program is funded primarily through family planning grants.

Rory Gibbens-Flores leads the team as Director of Outreach and Health Education. She is responsible for implementing SCHC's community outreach plan. Rory keeps track and makes sure SCHC is meeting the goals of various grants supporting the team's work. She reports to our various funding agencies and coordinates the Outreach Department's overall activities.

Vicente Lara's job as an outreach worker and health educator includes teaching about teen pregnancy prevention in local schools and working with young people and acts as the "adult ally" for Teen Clinic patients. More about Teen Clinic »

Olivia Ortiz is an outreach worker and health educator who makes presentations at local schools and various community agencies. She works with the twice-weekly family planning clinic at the Santa Rosa Junior College Health Center, and Olivia also makes sure SCHC has information available at Wednesday Night Markets and other local community events.

Abigail Barajas is our fearless Teen Services Coordinator who oversees
our Teen Advocacy Group (TAG) and promotes our teen services program, including Elsie Allen Health Center and Teen Clinic, in the schools and in the community. Abigail counsels students at Elsie Allen Health Center about their reproductive health choices and presents teen pregnancy prevention curriculum in local schools. More about TAG »

Cris Oseguera is our Gang Intervention Program Coordinator who created and launched Xinachtli: Young Men's Empowerment Program for young men at risk for gang involvement in Santa Rosa. More about SCHC's gang intervention program »
