  Chief Medical Officer

Southwest Community Health Center, a federally qualified health center in Santa Rosa, CA, is seeking a Chief Medical Officer. Reporting to the Chief Executive Officer, the CMO is a key member of the executive team whose primary role is to inspire and lead the clinical staff for the organization. The goal of the CMO is to ensure timely access to high quality medical care for the SCHC patients through clinical systems improvements, recruitment and retention of providers, supervision of site Associate Medical Directors, and oversight of the quality improvement initiatives. In addition, the Chief Medical Officer provides clinical expertise and leadership in the design and implementation of new projects and innovations at the Health Center.

Family Nurse Practitioner or Physician Assistant

The Physician Assistant or Family Nurse Practitioner provides comprehensive primary care to all patients seeking care at the Southwest Community Health Center under the clinical supervision of the Medical Director. S/he is responsible for maintaining medical records, follow-up and referrals, and participating in quality assurance activities. Telephone calls for patient advice and triage will be provided by the midlevel provider and/or physician on a rotating basis.

Clinical Diabetes Educator
(Registered Nurse and Case Manager)

The CDE/Case Manager, a 32-hours per week position, works with physicians, family practice resident, nurses, and dieticians to deliver care management services related to diabetes and other health problems. As a nursing team member, the CDE provides services including coordinating care and identifying and reducing risk factors. The CDE facilitates the disease management learning process for patients and their family members.

Bilingual Medical Assistant/Lead MA (Split Shift)

The Lead MA works with the Supervisor of Medical Assistants and/or Clinic Nurse Manager on a daily basis making decisions regarding backoffice patient flow and medical provide needs. This position's hours are split shift: Monday-through-Thursday 8 a.m.-12 p.m. and 6 p.m.-9 p.m., and Saturdays. The evening hours and Saturday shifts are Lead MA.
